To submit a product to the USGA for evaluation, simply complete the appropriate submission form below and submit it, with a sample of the product, to the address noted on the submission form. Effective for submissions received on or after April 1, 2009, the following charges will apply:

Please note that submissions will not be processed until payment has been received and verified. If paying by credit card, the USGA accepts American Express, Visa, MasterCard and Discover. Additionally, the USGA does not charge for opinions regarding the potential conformance of prototypes, mock-ups, or concepts of golf equipment technology communicated to the USGA.
