USGA Handicap System™ Golf Club Audit Checklist

Complete the following questionnaire and return to the United States Golf Association within 30 days of receipt.

1. What is the actual location of the golf club?—physical address and phone number is required.

 

2. Number of members receiving a USGA Handicap Index® from your club?

 

3. Does your club provide a Handicap Index to anyone who is not a member of the club? For example: Employees or others—please explain:

 

4. Does your club have a Handicap Committee composed mostly of members and chaired by a member? Please provide the name and mailing address of your Handicap Chairman for the current year.

 

5. Does your club follow the revision schedule and posting season of the authorized golf association having jurisdiction in the region? Please attach a copy of the revision schedule your club follows.

 

6. Does your club provide a reasonable and regular opportunity for its members to play golf with one another? Please provide examples of how this is accomplished and attach a copy of your club tournament schedule.

 

7. Does your club require the use of the USGA procedures to adjust hole scores before posting?

 

8. What is the name of the authorized golf association in your region?

 

9. Does your club ensure that all acceptable scores are entered correctly?

 

10. Does your club require the posting of all scores made at home and away?

 

11. Does your club make it possible for members to record the correct USGA Course Rating™™ and USGA Slope Rating® with each posted score from different set of tees?

 

12. What authorized golf association has rated your golf course within the past ten years? Please list the name of the golf association and the year the course was rated.

 

13. Does your club make current scoring records and a USGA Handicap Index® list available for inspection by others? Please provide examples of how this is accomplished at your club.

 

14. Does your club insist that the principles of The Rules of Golf be followed? Please attach examples of how this is accomplished at your club.

 

15. Please submit any other information that you believe would be helpful in assuring the USGA that your club is following the USGA Handicap System™™ in its entirety.
